Care Plan for Biliary Cirrhosis
Category: Autoimmune
Assessment
Patient presents with fatigue, pruritus, and jaundice.
Diagnosis
Risk for impaired liver function related to chronic inflammation and bile duct damage.
Goals
Patient will maintain liver function as evidenced by normal liver enzyme levels and absence of jaundice. Patient will verbalize understanding of the disease process and treatment plan.
Interventions
Monitor liver function tests regularly. Administer prescribed medications such as ursodeoxycholic acid to slow disease progression. Educate patient on the importance of medication adherence and avoiding hepatotoxic substances such as alcohol. Encourage regular follow-up appointments with a hepatologist.
Rationale
These interventions aim to manage symptoms, prevent complications, and ensure the patient is well-informed about their condition.
Evaluation
Patient's liver function and symptomatology will be reviewed during follow-up visits to assess progress and adjust the care plan as needed.
