Interventions

Monitor fluid intake and output, and provide diuretics as prescribed. Educate patient on dietary restrictions such as limiting sodium, potassium, and fluid intake. Encourage regular follow-up appointments with a nephrologist to monitor renal function. Provide resources for support services and nutritional counseling.
