Diagnosis

Risk for fluid volume deficit related to excessive gastrointestinal losses.

Goals

Patient will maintain normal fluid balance and have formed, regular bowel movements. Patient will verbalize understanding of dietary modifications to manage diarrhea.

Interventions

Monitor fluid intake and output, and assess for signs of dehydration. Administer prescribed antidiarrheal medications and monitor for side effects. Educate patient on dietary modifications, including the BRAT diet (bananas, rice, applesauce, toast). Encourage patient to drink oral rehydration solutions to maintain hydration.

Rationale

These interventions help manage symptoms, prevent dehydration, and promote normal bowel function.

Evaluation

Patient's fluid balance, bowel patterns, and understanding of dietary modifications will be monitored to evaluate the effectiveness of interventions.
