Care Plan for Peripheral Arterial Disease Management

Category: Cardiovascular

Assessment

Patient presents with intermittent claudication, leg pain, and diminished peripheral pulses.

Diagnosis

Ineffective tissue perfusion related to decreased arterial blood flow.

Goals

Patient will report reduced pain and improved ability to walk without discomfort. Patient will demonstrate understanding of lifestyle modifications to improve circulation.

Interventions

Administer prescribed antiplatelet agents and monitor for side effects. Encourage patient to engage in a supervised exercise program to improve circulation. Educate patient on smoking cessation and its impact on peripheral circulation. Monitor for signs of complications such as critical limb ischemia.

Rationale

These interventions aim to improve blood flow, reduce symptoms, and prevent complications associated with peripheral arterial disease.

Evaluation

Patient’s pain levels and ability to walk will be assessed and adjusted as needed during follow-up visits.
