Goals
Patient will maintain adequate oxygenation and demonstrate understanding of the importance of anticoagulant therapy.
Interventions
Administer prescribed anticoagulants and monitor for signs of bleeding. Monitor oxygen saturation levels and provide supplemental oxygen as needed. Educate patient on the importance of adhering to anticoagulant therapy and monitoring for signs of bleeding. Encourage patient to engage in early ambulation and leg exercises to prevent thrombus formation.
Rationale
These interventions aim to dissolve the thrombus, improve gas exchange, and prevent complications associated with pulmonary embolism.
Evaluation
Patient’s respiratory status and adherence to anticoagulant therapy will be monitored during follow-up visits.
