Assessment
Patient presents with paralysis, loss of sensation, and bladder and bowel dysfunction following a spinal cord injury.
Diagnosis
Impaired physical mobility related to spinal cord injury.
Goals
Patient will demonstrate improved mobility and independence with activities of daily living (ADLs).
Interventions
Encourage regular physical therapy to improve mobility and strength. Educate patient and caregiver on the use of assistive devices and adaptive equipment. Monitor for signs of complications such as pressure ulcers or urinary tract infections. Provide emotional support and refer to support groups and counseling as needed.
Rationale
These interventions aim to improve mobility, prevent complications, and provide support for patients with spinal cord injuries.
Evaluation
Patient’s mobility and ability to perform ADLs will be monitored during follow-up visits.
