Goals

Patient will demonstrate effective bladder emptying as evidenced by complete and regular urination. Patient will verbalize understanding of urinary retention management strategies.

Interventions

Monitor bladder distension and urinary output regularly. Administer prescribed medications to promote bladder emptying (e.g., alpha-blockers). Perform intermittent catheterization as needed to relieve retention. Educate patient on techniques to promote urination, such as double voiding and warm baths.

Rationale

These interventions aim to relieve urinary retention, prevent infection, and support bladder function.

Evaluation

Bladder emptying and urinary output will be monitored to assess the effectiveness of interventions.
